Behavioral Health & Employee Absence Management Nurse Case Manager

Duties:
The Nurse Case Manager - Hybrid (“NCM”) is an integral member of Axiom’s Behavioral Health (“BH”) and Employee Absence Management (“EAM”) Departments. The NCM will play a key role in performing telephonic case management for employee behavioral health and medical leave according to Axiom’s Standards Guidelines, applicable Nursing Directives and nursing best practices. The NCM will be accountable for consultation regarding mental health, medical leave, and return-to-work practices; keeping detailed records on employee communications, recording all necessary information in Axiom’s database, interacting with facilities as indicated, and properly communicating any pertinent/necessary information with Axiom’s clients. Additionally, the NCM will have other duties and responsibilities as determined from time to time by the Director of Behavioral Health & Absence Management.
Essential Functions:
The essential functions of the NCM are to:
- Perform and document telephonic mental health and medical leave case management according to Axiom’s Nursing Directives
- Follow client-specific directives for behavioral health screening, evaluation and triage
- Provide crisis intervention and stabilization efforts
- Complete medication reviews for employees who perform safety sensitive work
- Review medical leave documentation for FMLA, short-term disability and employment accommodations
- Uphold Axiom’s dedication to maintaining excellent client service and relationships by providing the highest quality of service, care, and expertise
- Stay current with regard to psychiatric-mental health and medication safety guidelines and case management procedures
- Educate workers, client representatives, and supervisors on Axiom’s style of case management, as needed
- Transfer cases appropriately
- Maintain relevant licenses in state of primary residence
- Obtain certifications/training as directed by Axiom
- Participate in departmental meetings and in-services
Qualifications:
The successful candidate should have an active RN, BSN or VN license in good standing plus a minimum of 5 years of demonstrated nursing experience with specific work experience in mental health or related fields. Previous nurse case management or telephonic case management experience is preferred.
Physical Requirements:
- Regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone keyboard reach,
- Specific vision abilities required by this job include close vision requirements due to computer work,
- Light to moderate lifting is required,
- This is a remote position and regular, predictable attendance is required.
